EXIT SURVEY / LEAVERS' SURVEY
Suffolk LEA has introduced a "Leavers' survey" (from January 2002) where members can give the LEA reasons for leaving your post with the LEA. You should get this form sent to you as a matter of course, when you give in your notice. If not, it is reproduced on this website. There are two parts:
Anonymous Survey Exit Questionnaire (.tif file)
Request for personal follow-up / interview or KITS register (.tif file).
Members are encouraged to fill in these forms frankly, particularly to flag up any issues which made your post difficult for you, and where work needs to be done to make improvements at the school, for the next postholder.
The form is anonymous, but the Union would encourage you to add the name of the school in the "Other comment" section, so that the LEA can be alerted to patterns at any particularly school.
If you have difficulty down-loading the tif files, ask the Secretary for a hard copy, giving your postal address. You can use the Feedback facility for this request or e-mail direct.
